What is Hospice Care? 

Hospice care is a type of health care that focuses on providing pain management and medical care to patients as they enter the last phase of their life. Hospice care focuses on treating the person and their symptoms instead of treating their disease.  

A team of medical professionals will evaluate and provide the care your loved one needs throughout their time in hospice. The medical team works with your family and loved one to create a custom care plan, ensuring the patient receives the kind of care they want. Medications and treatment focus on keeping the patient comfortable by alleviating their symptoms.  

Our hospice team delivers special services to make the end of a patient’s life more comfortable that include: 

  • Music therapy  
  • Massage  
  • Aroma and reiki therapies
 

These services are provided by board certified therapists trained in safe and effective physical and emotional relief.  

How Palliative Care is Different 

Palliative care brings the services of hospice to a larger population. You don’t have to be facing a terminal diagnosis to receive palliative care. The goal of palliative care is to provide pain management and compassionate care to patients at earlier stages in their illnesses.  

Insurances Accepted  

Affinity Care of Florida contracts with most insurance companies to cover the costs associated with palliative care or hospice care. We accept Medicare and many commonly held private insurances such as: 

  • All Medicare and Medicare replacement and HMO products
  • Aetna
  • FL BCBS
  • All Medicaid products and HMOs
  • Humana LTC

Affinity Care of Florida operate in conjunction with affiliated hospitals and skilled nursing facilities in the Florida area.   

  • Sarasota Memorial Hospital (all locations)
  • Doctors’ Hospital (all locations)
  • Any hospital who has a discharge returning to the Sarasota County

NURSING CARE

Regular visits by registered hospice nurses who have specialized training in pain and symptom management.

MEDICAL MANAGEMENT

Coordination of medical equipment and supplies for comfort and symptom management is managed by our competent staff.

HOME HEALTH AIDES

Visits by hospice home health aides provide additional personal care, time, and attention.

Emergency Care

Consultation and emergency care is available twenty-four hours a day, every day of the year.

Psychosocial Support

We provide psychosocial support for our patients, their families, and their long-term staff and caregivers.

Spiritual Support

Constant spiritual support is provided to both patients and their family members if requested

Special Therapies

Physical, occupational, speech, music, and other therapies indicated in the plan of care are all provided by our expert staff.

Volunteers

Trained volunteers who provide companionship and support for our patients and their families.

Bereavement Counseling

Bereavement counseling for family members is provided throughout the patient's illness and for a minimum of thirteen months following death.
